  1. Seven raw alarms resolve to one underlying failure and one decision row.
  2. The operating map explains what is physically connected.
  3. Evidence, operating limits, and the last verified fix arrive together.
  4. Every claim is marked grounded, partial, or unsupported; missing evidence stays visible.
  5. An engineer challenges the recommendation and the plan visibly changes.
  6. A named production foreman reviews and approves the action.
  7. A structured handoff is recorded while the Maximo connection remains in DRY_RUN. Nothing posts to a vendor system.
  8. Field work completing is recorded separately from recovery; until the signal is checked, the outcome stays unmeasured.
  9. In this demo world, recovery is then checked against the physical signal and written to the outcome ledger. A signal that does not return inside its band closes as not recovered.
